Position Summary 

The Pharmacist provides comprehensive clinical pharmacy services to home infusion patients at University of Iowa Community HomeCare, ensuring compliance with all applicable laws, regulations, and accreditation standards. This role is responsible for managing controlled substances, developing and implementing individualized care plans, monitoring and optimizing medication therapy, reviewing laboratory results, and collaborating with healthcare providers to ensure safe and effective treatment. The Pharmacist also plays a critical role in educating patients and caregivers, documenting clinical activities, monitoring adverse drug reactions, and maintaining seamless coordination of home infusion therapy. Additionally, the Pharmacist serves as a mentor and leader, coaching Pharmacy Technicians, Interns, and other staff to foster a culture of excellence and continuous learning.

Position Responsibilities

  • Accurately enter home infusion orders into the electronic health records (EHR) system, ensuring appropriate drug selection, dosage, administration route, and compatibility based on drug stability, patient needs, and clinical best practices.
  • Review, verify, and approve compounded and prepared sterile infusion products for accuracy, sterility, compliance with USP <797> guidelines, and adherence to safety protocols before dispensing.
  • Oversee and manage controlled substances, ensuring strict compliance with DEA regulations, inventory tracking, and proper storage.
  • Maintain oversight of inventory management, including monitoring stock levels, placing timely orders for medications and infusion supplies, and ensuring proper handling and storage conditions.
  • Communicate and collaborate with the IV Admixture staff to ensure appropriate preparation and delivery of home infusion products, including after-hours coordination as needed.
  • Provide direct patient and caregiver education regarding medication administration, potential side effects, and adherence to therapy plans.
  • Review and analyze patient laboratory results, clinical data, and medication responses, following up with prescribers and healthcare providers to make therapy recommendations.
  • Serve as a resource and mentor for pharmacy technicians, interns, and other staff, providing training and guidance on home infusion processes, compounding techniques, and regulatory compliance.
  • Assist in the development and execution of weekly compounding schedules to ensure efficiency in production planning and workflow optimization.
  • Monitor and report adverse drug reactions, medication errors, and drug-drug interactions, ensuring appropriate documentation and corrective action when needed.
  • Provide backup coverage for pharmacist colleagues, ensuring uninterrupted patient care during absences. Support the onboarding and training of new staff by providing guidance on home infusion processes, protocols, and best practices to ensure a smooth transition into the role.

Education and Experience Requirements

  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy and three years of applicable experience are required. 
  • Current licensure by the Iowa Board of Pharmacy Examiners is required. 

Desired Qualifications and Experience

  • Prior experience in home infusion pharmacy, sterile compounding, or clinical pharmacy services in an inpatient or outpatient setting.
  • Certification or advanced training in sterile compounding (e.g., BPS, USP <797>/<800> compliance training.)

Required Skills and Abilities

  • Strong written and verbal communication skills, with the ability to educate patients, caregivers, and healthcare professionals effectively.
  • High attention to detail and accuracy in medication verification, compounding, and documentation.
  • Ability to work collaboratively with multidisciplinary healthcare teams, including physicians, nurses, and home health providers.
  • Proficiency in pharmacy management software, electronic health records (EHR), and regulatory documentation systems.
  • Commitment to patient-centered care and service excellence in a home infusion setting.

Physical Requirements

These physical demands are representative of those that must be met by an employee to successfully perform the essential functions of the job.

  • Prolonged periods of sitting at a desk and working on a computer.
  • Work primarily in a cleanroom and/or pharmacy environment adhering to USP <797> and <800> standards for sterile compounding.
  • Use IV hoods, laminar airflow workbenches, automated dispensing cabinets, and infusion pumps as part of medication preparation and verification.
  • Lift up to 15 pounds at a time.

Additional Eligibility Requirements

  • Successful completion of background and reference checks.
  • Proof of eligibility to work in the United States.

Expected Work Hours and Location
