THE POSITION

Under general supervision of the Community Health Services Division (CHS) Deputy Division Director, the Community Outreach Worker (COW) assists in general outreach to priority populations in Alameda County. This position is focused on increasing equity in health outcomes in conjunction with CHS programs. Service activities include health education, assistance in case management, navigating systems, providing linkage to community-based organizations/agencies and referrals of eligible individuals and families to appropriate programs, and acts as a liaison between communities, agencies, and program staff. This position may also provide light case management by providing brief interventions for clients. A COW may also participate in community or health related coalitions and partnerships and may work with community-based organizations doing education in areas related to CHS programs. This position performs other service duties as assigned within the classification.

DISTINGUISHING FEATURES

Community Outreach Worker II is the journey-level class in the Community Outreach Worker series. Under general supervision, incumbents interact with clients and exercise independent judgment in making assessments and referrals based on their day-to-day social environmental interactions. This class is further distinguished from the higher professional classes (e.g. Nutritionist, Skilled Medical Professional) in that the latter performs at a higher level and with increased knowledge, technical skills and ability gained through experience and education. Professional staff also exercise significant judgment and make clinical decisions regarding client treatment and/or counseling services. This position will be trained across programs in the CHS Division.

EXAMPLES OF DUTIES

NOTE: The following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Each individual in the classification does not necessarily perform all duties listed.

  1. Performs community outreach and health education activities that support CHS program goals. Activities focus on priority populations and/or individuals in Alameda County with the universal goal of achieving equitable health outcomes across diverse communities.
  2. This position may include providing education, outreach and both organizational and client support in the following areas:

  1. Client navigation and referrals to programs serving priority populations. This may include:
  2. Provide individuals, families and community groups with educational information concerning health/social services programs in Alameda County.
  3. Inform clients of community services available and may contact those agencies and community-based organization on client's behalf.
  4. Assist clients in navigating the social and health services systems, including application assistance for MediCal and other public benefit programs that they are eligible for; screen individuals for eligibility to CHS programs; schedule appointments as needed.
  5. Assist professional and paraprofessional staff by conducting non-clinical interviews with clients and relatives and collecting client data regarding medical/social history and needs following established protocols; communicates these to professional or paraprofessional personnel as necessary; enters the data into program systems as requested.
  6. Direct phone outreach to newly enrolled participants in various CHS programs to assess health/social needs; make appropriate referrals as needed.

  2. Participating in community health fairs
  3. Conducting educational programs and classes in a variety of settings
  4. Public speaking at priority events
  5. Participating in community coalitions and partnerships
  6. Monitor public participation at attended events.

  1. Support communities, providers, businesses and organizations to adopt policies and change environments to improve community health through education, collaboration and advocacy. This may include:
  2. Working with CHS program staff to identify community leaders.
  3. Collaborating with Health Navigators and Community Outreach Workers from other programs.
  4. Educating community-based organization leaders and/or staff on a variety of topics to support changing environments and policies to improve health.
  5. Provides problem-solving support to individuals and families when public benefits/medical insurance status is disrupted.
  6. Attends staff conferences; represents staff in community/agency meetings.
  7. Translates CHS educational materials for the community as needed.
  8. Interprets information with community members as needed, including providing interpretation during client interviews, as needed.
  9. Prepares a variety of letters, memos and other written materials; accesses a variety of data bases and enters information to support program tracking.
  10. Regularly documents, enters and maintains records, databases, and/or tracking logs of community engagement interventions, services, and daily tasks per program protocols.
  11. Assists with case management services as assigned.
  12. Performs other duties as assigned within the classification.

COMMUNITY OUTREACH WORKER (COMMUNICABLE DISEASE)

MINIMUM QUALIFICATIONS

Experience:

The equivalent of one (1) year of full-time experience working in a community outreach program performing duties such as: Intake, peer counseling, obtaining personal history data, re-entry assessments, non-clinical assessments, providing client community health education, program screening, placement, and referral.

Substitution:

The equivalent of an AA degree (60 semester or 90 quarter units) from an accredited college in health services, social science health education or a related field, such as Psychology or Counseling, may be substituted for one (1) year of full-time experience in pattern II above.
